Sub standard quality embroidery

I am very disappointed with the quality of work received this time around. I have used the same logo multiple times on different items and gotten good quality results but this time something went very wrong.
The background colour was wrong and the embroidery wording was not done properly the R in the name seems to be unraveling.
I suppose the background color was approved by me but blue is very far from the purple that it should be.
